Please click this link to finalize your registration by signing the form. Both you and our clinic will receive a PDF copy of the completed form, officially registering you as our patient.

Before signing, we kindly ask that you carefully read through the third and fourth pages of the form, which cover important legal information and patient responsibilities. Our clinic is extremely active online, and we encourage you to explore our website and Facebook page, both of which are updated regularly with the latest news, health tips, and clinic information.

If you're unable to complete the form online, please arrive at the clinic about ten minutes before your scheduled appointment time to fill it out in person. Please note that we must have your completed form in order to proceed with your treatment.

If you haven’t booked your appointment yet, please note that bookings can only be made by phone or online. We do not accept bookings by email or messenger. If you choose to book by phone and the clinic is closed, please leave a message on our answerphone, and we will get back to you as soon as the clinic reopens. Please be aware that payment must be made in advance for initial appointments.
